First Visit

We are pleased that you have chosen us for your dental care and are committed to providing you with the highest quality of oral health care possible.

Your dental health is important to us.  You must be kept well informed about your oral health, so we would like to give you information about your first visit.

On your first visit you can expect:

  •  An introduction to our office and staff
  •  A thorough examination and review of your oral health, including necessary x-rays
  •  A careful evaluation of your dental status
  • A discussion of the most satisfactory treatment plan to meet your oral health goals

Please assist us by providing the following information before the time of your appointment:

  • Any x-rays taken by a previous dentist, if applicable.
  • Completed New Patient Health History Form, including a list of medications you are presently taking.
  • If you have dental insurance, please bring your card including insurance company address and phone number, employer name, and policy and subscriber numbers. This will save time and allow us to help you process any claims.

Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).
