Dental Insurance and billing
If you have dental insurance, please bring your insurance card with you or fax it to (919)782-1063. We accept all insurance; however Dr. Douglas is not a preferred provider (having no contractual obligation to any insurance company). We are considered out-of-network. This may or may not make a difference in your out of pocket costs. We are glad to file your insurance for you once all the necessary information is received and verified. However, in order to provide this service, your estimated co-payment and deductible will be due at the time of the appointment.

Please provide the following:
- Insurance company name, Claims address and phone number
- Policy or group #
- Subscriber ID # or Social Security #
- Employer Name
- If covered through spouse, include their social security number and date of birth
Payment options
- Flexible Payment Plan: Pay a portion up front, and the rest over an agreed upon time period. 1% interest will begin 60 days after date of service.
- Check or Credit Card: Always accepted. For your convenience we accept Visa, MasterCard, American Express and Discover.
Please note: Payment is due at the time of service.
As a courtesy to you, we will bill your insurance company and track claims. Please keep us informed of any changes to your insurance plan. You are responsible for the fees charged by our office, no matter what your insurance coverage may be. Most insurance companies should respond to the claim within four to six weeks. Any remaining cost is your responsibility. Select insurance companies will mail payments directly to the patient.
